We develop programs to provide the needed services and support to the identified needs in the community.
Based on the community health needs assessment, our community programming is planned by the community benefits advisory council and by the board of trustees when the three-year community health implementation plan is approved.
Our areas of community health priorities are:
- Access to healthcare
- Chronic disease
- Disaster readiness and emergency preparation
- Infectious disease
- Mental illness and mental health
- Preventable injuries and poisonings
- Social determinants of health
- Substance use disorders
- Housing stability and homelessness
- Violence and trauma
- Vulnerable populations
